Our office is a provider for most major insurance plans. We encourage you to call your insurance carrier to verify that our office participates in your plan or that you have out of network benefits.
Some insurance companies require a written referral from your primary care physician before you can be seen. You should call your insurance company to determine their referral requirements. If you have a completed referral, please give it to our receptionist when you arrive in our office.
Fees and Payments
We bill your insurance company within one week of your visit. If we have not recieved payment within 45 days, the balance will be transferred to your account for payment.
Our practice charges reflect what is usual and customary for our area. The charges for office visits and surgery are determined by the complexity of the problem being treated.
If you have insurance, we require payment of any co-pays at the time of your visit. If you do not have insurance, we expect payment for your medical service at the time of your visit unless prior financial arrangements have been made with our billing department.
We accept cash, check, Discover, MasterCard, or Visa.
Prescriptions and Refills
We cannot fill your prescriptions outside of our normal business hours since we do not have access to your medical records.
You must speak directly with a member of the staff to place your request.
The doctor may not be available immediately to authorize the request. This means you must anticipate your needs and call at least 48 hours prior to your medication running out.
